Two separate fees — two separate recipients. The clinic's consultation fee is paid at booking. The $150 ADHS state fee is paid separately and directly to ADHS through the ADHS Medical Marijuana Registry. The clinic does not collect any portion of the state fee. A $75 financial hardship fee is available for qualifying applicants.

After your appointment: Submit your application and written certification through the ADHS Medical Marijuana Registry. Pay the $150 state fee directly to ADHS. Once approved, your patient card is valid for one year at any licensed Arizona dispensary statewide. Purchase up to 2.5 oz per 14-day period.
